Camper Application and Medical Forms
Camper Applications must be submitted by applicants and confirmed in writing by Camping Unlimited before acceptance into programs. Medical Forms must be submitted prior to programs, after the Confirmation has been received.
Electronic forms (requires Adobe Reader):
*Please note that Camper applications may be filled out electronically and then either emailed or printed. Medical forms however may only be partially filled out and then printed to be completed by a physician. Both require use of the free Acrobat Reader, available at get.adobe.com/reader/. Adobe Reader must be installed on either a Mac or a PC to complete the forms electronically. Alternativly, the simple forms can be printed below, filled out by hand and submitted.
